CARDIOLOGY SERVICE EVALUATION:

Diagnoses:
1. Equivocal subaortic stenosis – peak gradient = 2.10m/s
2. Enlarged left ventricular internal dimension at end systole and diastole.

Presenting Problems/Complaints: Kyra, a 2 year old female intact German Shorthair Pointer, presents for an echocardiogram. She was previously diagnosed with a murmur by Dr. Strickland and an echocardiogram was recommended at that time to rule out subaortic stenosis.

Results of Cardiac Exam: Cardiac auscultation revealed a grade II/VI left basilar mid-systolic ejection quality murmur. No arrhythmias were noted on auscultation. A gallop rhythm was not noted. Femoral pulses were strong and synchronous.

Results of diagnostic evaluation:
Echocardiogram:
▪ Left ventricle: The left ventricle is mildly enlarged. Fractional shortening=35%. Left ventricular internal dimension diastole=47mm. Left ventricular internal dimension systole=30mm
▪ Left atrium: The left atrium is within normal limits. Left atrium to aorta ratio= 1.38 (2-D) and 1.12 (M-mode)
▪ Mitral valve: The mitral valve is within normal limits. No regurgitation is noted.
▪ Right ventricle: The right ventricle is within normal limits. No regurgitation is noted.
▪ Right atrium: The right atrium is within normal limits
▪ Tricuspid valve: The tricuspid valve is within normal limits
▪ Pulmonic valve: The pulmonic valve is unremarkable
▪ Aortic valve: Increased aortic flow velocity is noted with peak velocity = 2.10m/s. No aortic regurgitation is noted.
▪ Cardiac rhythm: A normal sinus rhythm was noted throughout the exam.

PLAN FOR RE-EVALUATION:

Yearly echocardiograms are recommended to monitor the systolic function.
